Again thank you for registering for Leading for Life. As part of your Pre-Assignment please complete the steps below.

1. Download Pre-Assignment Materials

You will find two different versions of the Pre-Assignment:

• Electronic: Download this PDF version of the Pre-Assignment if you prefer to work on it electronically. You will need Adobe Acrobat Reader DC software on your computer or a mobile device app compatible with PDF files to open and edit this file. Download electronic version.
• Print: Download and print this PDF version of the Pre-Assignment if you prefer to record hand-written responses. Download print version.

2. Complete DVULI Release Form

Please fill out the talent release form. DVULI may record and photograph the activites that will take place during the Leading for Life training. We need all participants to review and fill out the form. Complete the DVULI Release Form.
